ABSTRACT

OBJECTIVE:To optimize the formula of Captopril timing osmotic pump tablets. METHODS:Using accumulative release rate (Q) as index, single factor test was used to investigate the effects of blocking layer coating weight gain, semipermeable membrane coating weight gain,the type of polyepoxide (PEO),the amount of PEO (3 × 105) and HPMC in drug bearing layer,the amount of PEO (7 × 106) and NaCl in booster layer on drug release of Captopril timing osmotic pump tablets. Based on single factor investigation,using comprehensive score of release behavior(L)as index,the amount of PEO(3×105)and HPMC in drug bearing layer,the amount of PEO(7×106)and NaCl in booster layer as factors,L9(43)orthogonal test was used to optimize the formula of tablet core validation test was conducted. RESULTS:The optimal formula of tablet core included PEO(3× 105)71 mg and HPMC 15 mg in drug bearing layer,PEO(7×106)61 mg and NaCl 18 mg in booster layer,coating weight gain 7% and semipermeable membrane coating weight gain 10% in blocking layer. The osmotic pump tablet prepared by optimized formula released after 4 h;in vitro drug release regression equation was Q=5.118t-21.441(R2=0.9956),which was in line with zero-order release characteristics. CONCLUSIONS:The optimal formula is stable,feasible and controllable in quality,and can provide reference for further development of Captopril timing osmotic pump tablets.

ABSTRACT

Objective To evaluate the effectiveness and safety of minimal incision laparotomy assisted endoscopic resection for polyps in small intestine in patients with Peutz-Jeghers syndrome. Methods The clinical data of patients with Peutz-Jeghers syndrome, who underwent minimal incision laparotomy assisted endoscopic resection for polyps in small intestine, were retrospectively studied. The size and number of the endoscopically rosected polyps, and the procedure related complications were documented. Results A total of 812 polyps in small-bowel were resected in 8 patients, in which 384 were smaller than 10mm, 356 with diameter ranging from 11 mm to 30mm, and 72 were larger than 30mm in diameter. Procedure related complications included intestinal dysfunction in 1 patient and abdominal pain in another. No bleeding with a fall in Hb or other severe complications were observed. Conclusion Minimal incision laparotomy assisted endoscopic resectio is a safe and reliable procedure for the treatment of mid-small bowel polyps in patients with Peutz-Jeghers syndrome. It revolutionizes the therapeutic options for polyps in the region of the mid-small bowel and limits the indications for primary surgical management.

ABSTRACT

Objective To evaluate the value of miniprobe sonography (MPS) in diagnosing gastrointestinal submucosal protrusive lesions and selecting the indicated manual for treatment. Methods According to the sizes, properties, and depth of SMTs in the gastrointestinal tract detected by the MPS, different methods of resection were performed. Results Of 24 cases, 11 SMTs lying in submucosa under 2cm in diameter (2 benign gastrointestinal stromal tumors, 3 lipomas, 5 cysts, 1 granular cell tumor) were attempted with EMR or argon plasma coagulation (APC) ; there were no complications of hemorrhage or perforation. Thirteen SMTs lying in muscularis propria or with size of SMTs above 2 cm in diameter (4 malignant GISTs, 6 benign GISTs, 1 lipoma, 2 aberrant pancreas) were performed by surgical resection. Preoperative diagnoses of SMTs by MPS were consentient with their histological diagnoses. Conclusion MPS may detect the size, property, and depth of SMTs in the gastrointestinal tract and is helpful in selecting indicated cases for endoscopic resection. Endoscopic therapy of SMTs lying in mucosa or submucosa under 2cm in diameter is a safe and effective procedure.

ABSTRACT

One hundred and ninety-one rats were injured with blast super-pressure and 64 out of the 191 died within 30 minutes after injury.The characteristics of the myocardial damages of the rats were as follows:(1)Disturbance of myocardial circulation;(2)Air embolism and interstitial emphysema;(3)Degeneration and necrosis of the myocardium; severance of myocardial fibers ;and separation of intercalated discs.It was speculated that the causes of these myocardial pathological changes are the direct damage of blast superpressure and the secondary effects of pulmonary damages.Protection of the heart and the lungs from blast super-pressure injury is the key to reduce the number of immediate death in blast injury.

ABSTRACT

One hundred and forty mice, weighing 30-35 grams, were divided into irradiation group (12 Gy total body irradiation from a 60Co source), combined radiation-burn group (12 Gy total body irradiation and 15% TBSA third degree burns), and normal control group.The dynamic changes of the intestinal epithelium were observed with light and electron microscopes and autoradiography within 96 hours after injuries, and attention was paid to the formation and conversion of the misshapen cells. It was Jound that the proliferative cycle of the young cells in the intestinal crypts which were severely injuried by irradiation was significantly retarded, and their G1-phase to G2-phase were prolonged. These cells gradually changed into misshapen cells. Some of the misshapen cells could not divide and finally they wer.e desquamated; abortive proliferation was found in some of them; some others showed pathological karyokinesis. Thus polymorphic cells were formed. It was difficult to show that the misshapen cells could take part in the repair of the intestinal epithelium. It is speculated that the misshapen cells might to some extent have the function of absorption, secretion and barrier.

ABSTRACT

Seven hundred and forty-six mice, weighing 30-35 grams, were divided into four groups randomly. The animals of the first three groups were inflicted with radiation injury (12 Gy total body irradiation from a Co60 source) , burn injury (15% of third degree burns), and combined injury of radiation and burns(12 Gy total body irradiation and 15% of third degree burns) respectively. The fourth group worked as normal control.The mortality and the dynamic changes of the intestinal epithelium of the experimental animals were studied. It was found that the clinical course of the combined injury group was quite different from other two. The mortality of the combined injury group was higher and the damage on the intestinal epithelium was more severe than those of the radiation injury group in the first 48 hours after injury. In the second 48 hours, the mortality of the combined injury group became lower and the recovery of the intestinal epithelium faster.The reactions of the experimental animals to the combined injury of radiation and burn were-very complicate. Further investigation is essential to elucidate the mechanism and to formulate appropriate therapeutic measures.
